Medical Rates

We are pleased to share that there is minimal increase to the plan premiums for 2025. Please take a look at the documents below to review the per pay period and monthly premium amounts as well as the Summary of Benefits and Coverage for both plans offered.

• There will be only one change to the HDHP plan design, due to new ACA requirements.

  • The new Deductible/Out-of-Pocket Maximum will be $3300 for Single coverage. 

  • The Family Deductible/Out-of-Pocket Maximum will remain $6400. 

• The PPO Out-of-Pocket Maximum remains unchanged at $3000 for Single and $6000 for all other coverage levels.

​In addition to the 2025 Plan Comparison Chart (pictured below), we have created some basic scenarios for you to review to help visualize both the PPO and the HDHP annualized cost.​​​​

• Scenario 1 - Employee Only Coverage with minimal claim expectations

• Scenario 2 - Employee+Spouse Coverage, expecting a baby! 

• Scenario 3 - Employee+Family Coverage, with chronic condition

• Scenario 4 - Employee+Family Coverage, kids in sports + several office visits

• Scenario 5 - Employee Only Coverage, costly medication

• Worst Case Scenario under both the HDHP and PPO
