• Review the My Pet Protection Choice Plan Summary to explore:

  •  Accident coverage

  • Illness coverage

  • Wellness coverage for proactive care. 

 

Note: If you enroll by the 15th of the month, the coverage will be effective the 1st of the upcoming month.

 

Premiums will be deducted 24 times per year instead of 26.

2.  Call 877-738-7874 and mention you’re an employee of GreenState Credit Union

3.  Go to: PetsNationwide.com and enter GreenState Credit Union as your company name

How to Claim an Approved Reimbursement

Follow these steps once you receive an email from Nationwide/JP Morgan stating your claim is ready to be accepted.
